The way our district has helped define this to our staff was:
Tier One are sound teaching practices you would perform with any student.  The assessments are formative.  There is informal progress monitoring (i.e formative assessments, tests), and reteaching happens as necessary.
If a teacher consistently (3x a week in addition to the core) reteaches a concept/double-doses, or is monitoring the student's progress formally on a weekly or monthly basis, then it becomes tier 2.

For Tier 1, we've had teachers set instructional goals off of AIMS Web Data. At Tier 1, we've also implemented grade level meetings where teachers have time to look at the data, make instructional decisions, as well as discuss curriculum.  The time to meet and discuss data and resources is a huge part of success of Tier 1, as well as a good universal screening process with multiple data points.
